Responsible for the delivery of case management services to clients and their children. This position is responsible for the development and implementation of achieving independence. This includes all life domain aspects such as financial management, parenting, educational, social, physical, mental health, and spiritual.


  1. Responsible for the delivery of clinical and case management services to clients and their children.
  2. Monitors the placement of clients in the program, and develops holistic approach to providing service to clients. Responds to urgent issues as needed.
  3. Develops service plans designed to achieve permanency; implement plans and evaluate progress in accordance with DCFS rule and procedure.
  4. Arranges services in accordance with the service plan; works with paternal parents as appropriate. Receives and reviews progress reports concerning any services, educational, and/or job placements.
  5. Provides individual and/or group sessions with clients utilizing the Nurturing Parent Program and/or other evidenced based program as a guide.
  6. Maintains familiarity with DCFS policy and procedures as it applies to the foster care program, and assures compliance.
  7. Prepares for and participates in Child and Family Team meetings, Administrative Case Reviews, clinical staffings, IEP and other educational reviews and supervisory reviews; provides up to date information concerning case progress. Prepares for and attends court hearings on assigned cases.
  8. Completes and maintains case records in accordance with DCFS requirements, manner, including face-to-face contact, phone contact, and collateral contacts. Assures documentation is completed as defined in program standards.
  9. Arranges for or provides needed transportation to services.
  10. Attend training opportunities as required by DCFS or recommended by the Foster Care supervisor.
  11. Provides after hour emergency coverage as assigned.


  1. A Master's Degree from an accredited program in social work education, or in another human services field preferred, or
  2. A Bachelor's Degree in social work or a related human service field with supervision by a Master's Degree in social work or a related human service field with at least two years experience in the provision of foster care or other out-of­ home care.
  3. Licensure by DCFS or eligible for licensing.
  4. The ability to understand the issues of separation and loss for the child in care and a commitment to a child's right to beg to a family;
  5. The specialized knowledge, skills, and experience required to help the parents improve parenting capacities, maintain ties with their children, and develop and implement a stable, permanent plan for their future.
  6. The ability to provide service in an manner that reflects appreciation for the cultural background, heritage, and identity of each child.
  7. Good oral and written communication skills.
  8. Possess a driver's license in good standing and provide proof of insurance.
  9. Has reliable transportation.
